Meal Applications can be submitted online or a form can be obtained in the office!
Meal Applications need to be submitted before school starts, even if you qualified last year you have to requalify. School starts August 1, 2022. You can fill out an application online at family.titank12.com/application or click on TITAN logo on our district website. You are always welcome to request a paper copy from the district office please contact Elizabeth Flores at 928-668-5354 or email at email@example.com.

The letter must contain the complainant’s name, address, telephone number, and a written description of the alleged discriminatory action in sufficient detail to inform the Assistant Secretary for Civil
Rights (ASCR) about the nature and date of an alleged civil rights violation. The completed AD-3027 form or letter must be submitted to USDA by:
